Medical & administrative forms are required before participation in scuba diving activities or classes. The forms below relate
to courses available through Splash Dive Center, Inc. Please download and complete the forms indicated by your course instructor,
and return them to him or her at your next class session.
Open Water (Beginning) Scuba classes require 3 forms including Medical, Safe Diving Practices, and Class release form.
For diving students under the age of 18, all forms must be signed by a parent or legal guardian.
If you answer "Yes" for any item on this form, a physician's signature is also required prior to in-water activities.
